Memorial Garden Interment Request

The Plymouth Memorial Garden is intended for the interment of the cremated remains of Plymouth Church members or the immediate family of Plymouth members.

Prior to any interment of remains in the Garden, this request MUST be completed by a family member, and its submission will constitute a legal release to inter the cremains, and serves as acknowledgment that all interments into the Plymouth Memorial Garden are irrevocable.

Only the names of those whose remains have been interred may be engraved on the Memorial Plaque. The precise name to be engraved MUST be specified below.

In consideration of the costs associated with engravings the Memorial Plaque, preparation of the interment site, and ongoing Garden maintenance, a minimum contribution of $250 is required.
